Diabetes Program

The Diabetes Program is accredited by the American Diabetes Association and is funded through Special Diabetes Programs for Indians (SDPI) Grant. The diabetes team is comprised of certified diabetes care and education specialists, registered dietitians, registered nurses and administrative staff. It serves individuals newly or previously diagnosed with diabetes in varying stages of life, through a variety of individual and group sessions with the mission to empower individuals to manage their diabetes and prevent associated complications by promoting healthy living.

Services

  • Patient Education – Individual & Group Education
    • Building Education About Diabetes (BEAD) is a 5 session group program featuring our new curriculum designed for those newly diagnosed with diabetes or those needing a refresher course in the management of diabetes.
  • Nutrition Education and Counseling
  • Cooking Demonstrations
  • Diabetes Technology Program
  • Medication Assistance (instruction, management, and support)
  • Foot Care
